Can Any Supplement Prevent Dementia? What the Big Trials and the Law Say

Ads promise protection from Alzheimer's. The trials, the law and the risk-factor research tell a different story, with better news in it.

The short answer

No dietary supplement has been shown to prevent dementia in a large, well-run trial. The most thoroughly tested, ginkgo biloba, was given at 120 mg twice a day to 3,069 adults aged 75 and older for a median of about six years in the GEM study; it did not reduce dementia or Alzheimer's disease[1]. Under U.S. law a supplement may not even claim to prevent a disease[4], so any ad that promises protection from Alzheimer's has stepped over a legal line as well as an evidential one.

What does reduce risk

The 2024 Lancet Commission estimates that about 45 percent of dementia cases worldwide are linked to 14 modifiable risk factors, among them hearing loss, high LDL cholesterol, high blood pressure, physical inactivity, smoking, diabetes, depression, social isolation, excess alcohol, air pollution and untreated vision loss[2]. The FINGER trial showed that a two-year program of diet, exercise, cognitive training and vascular monitoring improved cognitive scores in at-risk older adults[3]. The National Institute on Aging's guidance on cognitive health follows the same lines[6].

How to spot an overreaching claim

  • "Reverses memory loss" or "rebuilds your brain".
  • "Protects against Alzheimer's" or "prevents dementia".
  • A doctor or scientist whose credentials cannot be checked.
  • "Breakthrough" research that is not cited, or that turns out to be in mice.
  • Urgency: countdowns, limited stock, "price going up".

The FDA's health fraud guidance lists these patterns for products aimed at serious conditions[5].

Where supplements fit

A supplement can be a reasonable personal experiment for ordinary age-related forgetfulness, chosen with a label you can read, checked for interactions and judged by a log. It is not prevention, and it should never displace the measures above.

Quick answers

Has any supplement been proven to prevent dementia?

No. Ginkgo, the most tested, failed in a six-year trial of 3,069 older adults.

What lowers dementia risk?

Managing hearing loss, blood pressure, cholesterol, diabetes, inactivity, smoking, depression and isolation; about 45 percent of cases are linked to modifiable factors.
